Our Covid Policies & Procedures are available to read.
Riders must have our updated 2024 forms completed by their doctor and parents in order to ride.
- Rider Application/Health History - completed by parent or guardian for each rider each year
- Rider Physician Form - to be completed by doctor or health care professional
- Rider Waiver Form - to be completed for each rider each year
- Rider Emergency Medical Release - to be completed for each rider each year
- Volunteer Handbook and Forms - all volunteers must read and sign all forms within the handbook. If volunteer is a minor, parent or guardian must also sign all forms.
